280811 saya keje nite shift, alhamdulillah nite shift relek jer. dapat pulak ms kiren yang sangat lah OK. dia tak tido pon. sesetengah pegawai suka gilir-gilir. tapi ms kiren ritu tak pon. dia allocate + filling dekat blakang, saya regist + dispen dekat depan (^_^) --> i do hate filling. sebab malaih la nak cari-cari ubat (-_-')

since tak baper busy sangat, saya baca la buku-buku yang ada dekat kaunter tu (next time nak bawak novel la.ini tak, abis suma buku ubat-ubat saya bukak.haha)... ade sorang 'cikgu' suruh saya ingat doses untuk paeds sebab some pegawai suka tanya... huuu~~ baeklah~!!
  1. Amoxy + Clavulanic 228mg/5mL [Augmentin] --> Recommended dose : 25 - 45 mg/kg/day BD

  2. Ampicillin + Sulbactam 250mg/5mL [Unasyn] --> Recommended dose : 25 - 50 mg/kg/day BD (<30kg), if more than 30kg used adult dose 375 - 750 mg BD

  3. EES 200mg/5mL --> Recommended dose : 40 mg/kg/day BD or QID. Up to 100 mg/kg/day in severe infection.

  4. Sulphamethoxazole 200mg + Trimethoprim 40mg /5mL [Bactrim] --> Recommended dose : 1.5 - 3 mg/kg/day BD

  5. Cloxa Na 125mg/5mL --> Recommended dose : 50 - 100 mg/kg/day QID (<20kg), if more than 20kg, 250 - 500 mg QID. ***only for >1month old.

  6. Phenoxymethylpenicillin 125mg/5mL [Penicillin V] --> Recommended dose : 25 - 50 mg/kg/day TDS or QID (<12 years old). ***the dose not include prophylaxis of pneumococcal infection + recurrent rheumatic fever.

  7. Cefuroxime 125mg/5mL [Axetil] --> 10 mg/kg/day BD, max : 250mg daily. Otitis media : 15 mg/kg/day BD. Severe infection : max 500mg daily.

  8. Nystatin 100 000 units/mL --> Recommended dose : 1 mL QID for infant, 1 - 6 mL QID for child and adult

  9. PCM 120mg/5mL [Panadol] --> Recommended dose : 15 - 20 mg/kg/day QID depending on condition.

  10. Chlorpheniramine maleate 2mg/5mL [Piriton] --> Recommended dose : 1 mg BD (1 - 2 years old), 1 mg 4 to 6 hours max 6 mg (2 - 5 years old), 2 mg 4 to 6 hours max 12 mg (6 - 12 years old).

  11. Bromhexine 4mg/5mL [Bisolvan] --> Recommended dose : 1.25 mL TDS (<2 years old), 2.5 mL TDS (2 - 6 years old), 5 mL TDS (6 - 12 years old)

  12. Salbutamol 2mg/5mL [Ventolin] --> Recommended dose : 0.1 mg/kg QID (<2 years old), 1 - 2 mg every 6 to 8 hours (2 - 6 years old), 2 mg every 6 to 8 hours (6 - 12 years old)

  13. Diphenhydramine HCl Expectorant [Benadryl infant] --> Recommended dose : 5 - 10 mL QID. it is best for child < 6 years old.

  14. Benadryl adult --> Recommended dose : 2.5 mL QID (1 - 5 years old), 5 mL QID (6 - 12 years old), 5 - 10 mL QID (>12 years old).

  15. Promethazine HCl 5mg/5mL [Phenergan] --> Recommended dose : 5 - 15 mg daily (2 - 5 years old), 10 - 25 mg daily (5 - 10 years old).

  16. Metoclopramide HCl 5mg/5mL [Maxolon] --> Recommended dose : 1 mL BD (<1 year old), 1 mL BD or TDS (1 - 3 years old), 2 mL BD or TDS (3 - 5 years old), 2.5 - 5 mL TDS (> 5years old).

  17. Prednisolone 2.5mg/5mL --> Recommended dose : 0.14 - 2 mg/kg/day QID (< 12 years old)

  18. Multivitamin Syr --> Recommended dose : 5 mL OD for Tx and Px of vitamin deficient

  19. Oseltamivir 12 mg/mL [Tamiflu] --> Recommended dose : 30 mg BD, (< 15kg), 45 mg BD (15 - 23kg), 60 mg BD (23 - 40kg), 75 mg BD (> 40kg)

  20. Albendazole 200mg/5mL [Zentel] --> Recommended dose : 200 mg single dose (1 - 2 years old), 400 mg single dose (> 2 years old)

  21. Tripolodine HCl 1.25 mg + Pseudoephedrine 30 mg / 5mL [Actifed] --> Recommended dose : 2 - 5 mL TDS (2 - 5 years old), 5 mL TDS (6 - 12 years old). Contraindicated in < 2 years old.

  22. Loratadine 1 mg/mL [Clarityne] --> Recommended dose : for 2 - 12 years old, 5 mL OD (< 30kg), 10 mL OD (> 30kg). Contraindicated in < 2 years old.

  23. Hyosine N - Butylbromide 1 mg/mL [Buscopan] --> Recommended dose : 10 mL TDS (6 - 12 years old). Not recommended for < 6 years old unless justified by Doc.
em3,,, syrup yang common --> PCM, Penicillin V, Benadryl... ini je kot yang salu bagi... Penicillin V tu yang kene dilute dulu tu, perlu aja sikit parents tu sebab kita just dilute 1st bottle je kan...
ms kiren cakap,,, kite ni Pharmacist, bila dispen tu kena aja patient. bukan setakat cakap yang ni mkn sebijik 3kali / 10mL dua kali sehari~~~ huhu (^_^)

antibiotic Stability~

orait. table dekat bawah ni adalah antibiotic yang available di HSB.

hari tu (tak ingat dah hari ape tapi the same day saya buat syrup morphine tu), Puan Kiren dekat Satelite 6 bagi assignment nih. die suruh bace sume leaflet untuk sume antibiotic yang ade, cari stabiliti after reconstitution. means once dah bukak, antibiotic tu bole tahan lame mane... Puan Kiren mintax buat bende ni sebab ade sesetengah supply adalah 'cap ayam' --> x brape elok kualiti (produk2 ceni meningkatkan pembaziran sebab tak tahan lame.huhu).

tapi,,, saya baru bace 2 leaflet, encik Edi dari production suruh turun buat syrup morphine. huhu~ DD memang sume bagi dekat PRP, since saya je sorg PRP baru dekat IPD time tu, saya la kene buat huhu~ xpe. learning a new thing xkan penah merugikan (^_^)

so,,, thanx to adik-adik (nabilah+pavi) yang tolong siapkan assignment kak amal nih!~ (^_^)v hehehe



oh ye, due to the case of resistance, some antibiotic required special form kalau nak indent. form tu kaler pink.

antibiotic yang terlibat:
Meropenem, Imipenem, Ertapenem, Vancomycin
...
supposedly kalau Dr indent any of these 4, WITHOUT form kaler pink tu, kite x bole bagi... tapi tengok atas budi bicara la jugak rasenye... kalo dah case tu urgent, terpaksa la bagi tapi tulis la note dekat label tu 'please send abx order form within 72hours'
keempat-empat antibiotic nih perlukan countersign...
rase nye in future akan ditambah lagi senarai tu sebab ade new case of resistance... cephalosporin pny grup. tapi tak ingat ape ntah...

Recepi Sirap Morfin~

ehem3~ lama nye tak update blog hee~~~ (^^)v

ok. harini 15 Ogos 2011, sudah masuk minggu ke-3 saya berkhidmat di Hospital Sultanah Bahiyah, Alor Setar...
malas la nak intro panjang-panjang sebab ade keje nak buat lagi.

straight to the point,,,
nak buat rekod dekat blog pasal formula membuat Aqueous Morphine 10mg/5mL (ataupun 2mg/mL)...
p/s: formula Aq Morphine antara soalan viva untuk inpatient ktorg.

ALATAN:
Silinder penyukat, Spatula, Corong turas

BAHAN MENTAH:
Morphine HCL BP, Simple Syrup BP, Chloroform water BP qs

MASTER FORMULA:
  1. Morphine HCL BP 4000mg
  2. Simple Syrup BP 500mL
  3. Chloroform water BP qs 2000mL (dekat production HSB pakai chloroform water duplex which means concentration die double. so kena dilute la dulu, 1:1 dengan distilled water.)
***master formula ni untuk 2L. tadi saya kena buat untuk 10L... so kira la yek bape quantity untuk each.
***tapi,,, cane nak explain portion syrup dengan chloroform tu eh? haha tapi tadi saya guna 20g serbuk morphine, 2500mL syrup, 3750mL chloroform water, 3750mL distilled water.

PROSEDUR KERJA:
  1. Timbang serbuk morfin yang diperlukan.
  2. Cairkan chloroform water duplex kepada chloroform water simplex dalam satu container bersih.
  3. Masukkan serbuk morfin ke dalam kontainer mengandungi chloroform water simplex dan kacau sehingga sebati.
  4. Masukkan sirap ke dalam campuran morfin dan chloroform. Kacau sehingga sebati.
  5. Masukkan aqueous morphine yang telah siap ke dalam bekas yang sesuai dan labelkan.
***since ni DD, so everything pharmacist yang kene buat, filling ke labeling ke tak bole PPF yang buat. so as PRP, DD nih memang keje kite la yek huhuhu~
***tadi saya buat 5 botol of 1L. yang 1L ni untuk farmasi pesakit dalam (FPD). Then, 24 botol of 240mL, yang ni untuk farmasi klinik pakar (FKP).

orait. senang je nak penuhkan extemporaneous prep punya requirement. banyak je dapat buat. Syrup Clonazepam la paling banyak... (^_^)
